Labels are very important on forms. It is best to use as few words as possible to allow users to quickly scan the form. With short labels, it is fine to use title case, however, for longer labels, it is best to use sentence case as a sentence in title case can be difficult to read. All caps should be avoided as it makes the form difficult to read and harder to scan. Labels should be at the top of their fields. It is the best for allowing users to scan the form as quickly as possible.

It is important to keep a form straightforward and easy to navigate. Asking too much information can decrease coversion rates and submissions. Only the most crucial information should be asked for. To increase conversion, it is also best to make sure the form is only one column as it is easier to follow, thus also increase conversion. Finally, forms should be in order form easiest questions to harder questions. Visitors are more likely to finish what they started, even if they have just entered their name. Time consuming questions at the start can dissuade visitors from filling out the form.
